General Workforce Package
General hires with physical job demands
- Non-DOT physical ($115)
- Vision test, Snellen ($43–57)
- Optional: Standard criminal package ($25.95)
We’ve partnered with EScreen to provide occupational health screenings, including DOT and non-DOT physicals, audiograms, vision tests, titers, vaccines, and more. Individual orders have no minimum or contract requirement.
Pick a package based on each worker’s job duties and physical demands. Role requirements, facility policies, and state rules may call for additional services.
General hires with physical job demands
Warehouse and equipment-operator hires
Roles where the employer requires respiratory screening
Healthcare hires needing employer-selected TB screening
Clinical staff needing employer-selected immunity and TB screening
Keep screening after the hire. Each month we re-run OIG LEIE, GSA SAM, and OFAC searches to flag newly reported exclusions or sanctions. You set the screening frequency your laws, payers, and policies require.
Build an order with exactly the services a role requires.
A general physical exam for hires with physical job demands, performed at an EScreen network clinic.
DOT physicals and recertifications performed by a qualified medical examiner for drivers in DOT-regulated roles.
EScreen and Labcorp drug test panels, DOT drug tests, and breath alcohol tests, ordered separately from physicals.
A functional lift test for warehouse, equipment, and other physically demanding roles.
Hearing tests for roles with noise exposure or hearing requirements.
Snellen, Jaeger, Ishihara, and Titmus vision tests matched to the role’s requirements.
OSHA respirator questionnaires, spirometry, and qualitative or quantitative fit testing.
Hep A, Hep B, Hep C, MMR, mumps, rubella, and varicella titers to confirm immunity.
TB/PPD skin tests, QuantiFERON and T-SPOT blood tests, and chest X-rays when clinically indicated.
Hepatitis A and B, MMR, varicella, influenza, and Tdap vaccines for healthcare and clinical roles.
Individual service prices through EScreen and Labcorp. Combine services into a package, and we’ll confirm the clinic location and turnaround with your quote.

DOT services are separate from non-DOT tests. A drug test, alcohol test, and physical are separate services, and ordering them does not by itself establish DOT compliance.
Occupational health packages for general, warehouse, respirator-use, and healthcare roles, with an itemized quote for each.
Request a quote for the package that fits each role. We’ll confirm the itemized price, estimated turnaround, and EScreen clinic location before you order.
Clinical hires often need TB screening and proof of immunity before patient contact. A package may combine a TB/PPD skin test or TB blood test with Hep B, MMR, and varicella titers, plus the vaccines a facility requires.
Pair occupational health services with license verification and OIG and GSA exclusion checks, and add the Monthly Exclusions Check to monitor staff after the hire.
Warehouse and equipment roles often combine a lift test, an audiogram for noise exposure, and a drug screen. Add a non-DOT physical or vision test when the job’s physical demands call for one.
Add the Standard criminal package for $25.95 per report if the role requires criminal searches.
OSHA requires a medical evaluation before an employee uses a respirator. A package typically starts with the OSHA respirator questionnaire and spirometry, followed by a fit test.
Fit testing is priced separately. Choose a qualitative or quantitative test based on the respirator the employee will wear.
DOT-regulated drivers need a DOT physical from a qualified medical examiner, plus DOT drug and alcohol testing where the role requires it. Each is a separate service, ordered individually.
For driving history, CDL verification, and FMCSA checks, see our Transportation packages.

No. A drug test checks a specimen for specified substances. A physical or fit-for-duty exam assesses health or ability to perform job duties and requires clinical evaluation.
No. Screening supports your hiring procedures, but employers remain responsible for determining and meeting the requirements for each role.
Yes. Combine criminal, driving-record, license, exclusion, occupational health, and drug-testing services based on each role’s requirements, and we’ll quote the resulting package.
EScreen offers DOT and non-DOT physicals at participating clinics, and we’ll confirm the nearest location and qualified examiner when you order. Ask our team about fit-for-duty exams. A DOT physical does not include a DOT drug test; order each separately.
Most occupational health and drug testing results return in 2–5 business days. Turnaround times are averages, not guarantees; clinic scheduling and the services ordered can affect timing.
Yes. 5 Star Background Checks is able to sign a HIPAA Business Associate Agreement when the services and data involved require one. We are HIPAA compliant and SOC 2 Type II audited.
